EasyCare, Inc. Testimonial Submission Terms and Photography Release

Photography Release

By submitting your photo, you give EasyCare, Inc., of Tucson, Arizona, the absolute right and permission to use your photo(s) in its promotional materials and publicity efforts. You understand that the photographs may be used in a publication, print ad, direct-mail piece, electronic media (e.g. video, CD-ROM, Internet/WWW), or other form of promotion. You understand that the images may be altered and you waive the right to approve of any finished product. You release EasyCare, Inc., their employees, agents, and designees from liability for any violation of any personal or proprietary right you may have in connection with such use. You certify that you are over 18 years of age and that you have the full legal right to execute this agreement.

 

Your submission of the photo is your guarantee that you are the owner of the photo. Photographers retain the copyright to their photographs.

 

Testimonial Submission Terms

By submitting your testimonial, you give EasyCare Inc., its employees, agents, and designees full permission to reproduce and display the testimonial in any form, and to use it in any promotional materials and publicity efforts. You understand that EasyCare Inc. may, at its discretion, edit your testimonial for spelling, grammar, or wording when necessary. You understand that submitting a testimonial is not a guarantee that it will be used or published, and that EasyCare may remove or delete a testimonial at any time, without notice.
